SERVICES OFFERED:

 

  1. EMERGENCY ASSISTANCE

Requirements:

· Medical Assistance:

               -Original copy of Medical Abstract/Medical Abstract of patient.

               -Original copy of prescription/laboratory request.

               -Original copy of Certificate of Indigency signed by Punong Barangay/OIC.

               -Valid I.D. of client.

· Food Assistance:

               -Original copy of Certificate of Indigency signed by Punong Barangay/OIC.

               -Valid I.D. of client.

· Transportation Assistance:

               - Original copy of police blotter for victims of pick pocket/strandees.

               -Valid I.D. of client.

               -Original copy of Certificate of Indigency.

               -Referral letter of patient.

               -Death Certificate of relative for those who will attend burial.

               -Subpoena for minors attending court hearing.

1.2 BURIAL ASSISTANCE

Requirements:

Original and certified true copy of:

1. Death Certificate of deceased indigent.

2. Any valid government I.D. of claimant.

3. Senior Citizen I.D. if senior citizen.

4. Certificate of Indigency from the barangay.

5. Funeral contract.

1.3 EMERGENCY SHELTER ASSISTANCE (ESA)

 Requirements:

1. Police blotter or certification from the barangay of the fire incident.

2. Valid I.D. of claimant.

3. Pictures of burned residential house.

2. ISSUANCE OF SOLO PARENT I.D.

Requirements:

1. Death Certificate of spouse for widow/widower.

2. Certificate of a public medical practitioner for mentally ill/physically handicapped spouse.

3. Court Order for legally separated or in jail.

4. Birth Certificate of minor dependents.

5. Certification from Punong Barangay.

6. Two (2) pieces of 1x1 picture.

3. ISSUANCE OF CERTIFICATE OF INDIGENCY

Requirements:

1.  Certificate of Indigency from the barangay.

2.  Valid ID of client.
